Submitting a request does not automatically bind, change, replace, or cancel insurance coverage. A requested change is not effective until it has been confirmed by McCloskey Advisor Group or your insurance company.
PROTECT YOUR INFORMATION
Do not submit Social Security numbers, complete payment-card numbers, online account passwords, or other highly sensitive information through this form.
URGENT OR AFTER HOURS HELP
For urgent claims, billing issues, or assistance outside regular business hours, contact your insurance company directly using the telephone number on your insurance card or policy documents. Call 911 if anyone is injured or immediate emergency assistance is needed.

What Do After a loss
Make Sure Everyone is Safe
Move to a safe location when possible. Call 911 if anyone is injured, there is a fire, a crime has occurred, or emergency assistance is needed.
Prevent Further Damage
Take reasonable steps to prevent additional damage when it is safe to do so. Keep receipts for temporary repairs or emergency services.
Document What Happened
Take photographs, make notes, collect contact information, and obtain a police or incident report when appropriate.
Report the Claim Promptly
Contact your insurance company as soon as reasonably possible. Use the claims telephone number shown on your insurance card, policy documents, or insurance company website.

No. Submitting an online form, email, voicemail, or text message does not automatically bind, change, replace, or cancel coverage. The request must be reviewed and confirmed before it becomes effective.

We review service requests during regular business hours and respond as soon as we reasonably can. Contact your insurance company directly when immediate or after-hours assistance is required.

You may be able to download an identification card through your insurance company’s website or mobile application. You can also submit a document request through this page.

Submit a service request and include the certificate holder’s legal name, mailing address, email address, and any written insurance requirements. Coverage or wording that is not included in your policy cannot be added to a certificate without approval.
